How to create & format d1m, d2m and d4m disk images

How to create & format d1m, d2m and d4m disks using just VICE (2.4.8 linux, with the FD-2000 v1.40 & FD-4000 v1.40 roms) & FD-TOOLS (v1.06).

Below are the detailed steps, but the tl;dr version and general tip is that whenever you attach a d1m, d2m or d4m disk in VICE, you need to reset the drive you've attached it to AFTER you attach the disk. I'm not sure where the bug is or why this works, but after some experimentation, this seems to be the way to get these images to work consistently in VICE.

I've uploaded the files created, for convenience:

To make a blank d1m (or d2m or d4m) disk:
Start: x64
File/Create and attach an empty disk/Unit #9

Create empty disk dialog:
Name: test.d1m (or d2m or d4m)
Disk options
- Disk name: (leave blank) ID: (leave blank)
- select d1m (or d2m or d4m) radio button
Save
- VICE will exit (Received signal 11 (SIGSEGV), exiting.)
but disk will be created

Start: x64
Settings/Enable Virutal Devices disabled
Settings/Drive settings/True drive emulation enabled
Settings/Drive settings/Drive #8 model/1541-II
Settings/Drive settings/Drive #9 model/2000 (or 4000)
File/Attach disk image/Unit #8/fd_2000utils.d64 (ie, disk where FD-TOOLS is)/Open

in Commodore 64:
LOAD"FD-TOOLS",8
RUN

in FD-TOOLS (you should see DEVICE: 9 TYPE/DENSITY: NO DISK):

DISKETTE FORMATTING OPTIONS:
- TYPE: CMD
- DENSITY: DD 800K (or HD 1.60M or ED 3.20M)
- PARTITIONS: 1 NATIVE
- NAME: (one space)
- ID: (two spaces)

when you see "PLEASE INSERT DISKETTE...":
File/Attach disk image/Unit #9/test.d1m (or d2m or d4m)/Open
*** IMPORTANT! *** Do not press Return in FD-TOOLS yet...
File/Reset/Unit #9
- if you do not reset the drive after attaching the disk, you may
get a warning similar to the following in FD-TOOLS:

warning!
a disk error has occurred:
error #76 on track 1 sector 0
hardware error

and you will then have to start over in FD-TOOLS

Return to begin FORMATTING DISKETTE...
- Alt-w for warp mode, if desired

Exit FD-TOOLS

To see blank formatted disk:
LOAD"$",9
LIST
